About the Opportunity

The Registered Nurse (RN) - Home Health is a key member of our interdisciplinary team, responsible for providing high-quality, patient-centered care to individuals in the comfort of their own homes.

As a skilled nursing care professional, you will work collaboratively with our team of healthcare professionals, including physicians, therapists, and social workers, to develop and implement comprehensive care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans to ensure optimal health outcomes
  • Provide skilled nursing services, including wound care, medication management, and IV therapy
  • Collaborate with patients and their caregivers to educate them on disease management, prevention of complications, and self-care
  • Document patient care in a timely and accurate manner
  • Participate in quality assurance and performance improvement activities to ensure the highest level of care

Requirements:

  • Active Florida RN license
  • At least 1 year of nursing experience, with Home Health experience preferred
  • Ability to work independently with minimal supervision
  • Strong clinical and critical thinking skills
  • Excellent communication and interpersonal skills
  • Reliable transportation
