Position Overview

PeaceHealth is in search of a Registered Nurse - Palliative Care for a Per Diem/Relief, 0.00 FTE, Evening position. The general salary range for this role at PeaceHealth is $43.88 – $69.54, with the actual hiring rate influenced by various factors such as job responsibilities, location, and the applicant's qualifications.

This role entails delivering direct skilled nursing care and overseeing the management of patients receiving home care or clinic services, employing significant judgment and expertise in patient care decisions. The professional healthcare process involves assessment, problem identification, planning, intervention, and evaluation of clinical outcomes.

KEY RESPONSIBILITIES:

  • Conduct initial evaluations of patients and their homes, applying admission criteria to assess safety for home health or hospice services.
  • Administer skilled nursing care and provide education to patients and caregivers in accordance with the established care plan.
  • Perform comprehensive physical assessments, develop, coordinate, and adjust individualized care plans as prescribed by the primary physician to achieve both short and long-term medical objectives.
  • Manage and adapt the Home Health Aide's care plan based on the patient's changing personal care requirements.
  • Facilitate care coordination, collaborating with interdisciplinary team members to ensure seamless service delivery throughout the care continuum.
  • Implement discharge planning tailored to patient-specific goals and assess the effectiveness of interventions and outcomes based on established care objectives.
  • Ensure accurate, complete, and timely clinical documentation in line with regulatory and quality standards.

QUALIFICATIONS:

  • Graduation from an accredited School of Nursing is required; a Bachelor's Degree is preferred.
  • A minimum of 2 years of experience in acute clinical or home health/hospice nursing is essential.
  • Must hold a Registered Nurse license in the state of Oregon.
  • Possession of a valid Driver's License and proof of auto insurance is required.
  • Basic Life Support certification from the American Heart Association is mandatory.

Note: This position is governed by a collective bargaining agreement, and there may be multiple openings available.
