Job Description

As a Case Manager at ActOne Group, you will be responsible for assessing veterans referred to the program, identifying needs and barriers in housing, employment, education/training, and financial areas. You will coordinate care with VA personnel and community partners to secure eligibility documentation and mental and physical health services.

You will facilitate the client intake process, ensuring all program eligibility documentation is complete. Additionally, you will collaborate with clients to develop and complete Individual Action Plans. Your attention to detail will ensure confidential client files are maintained, adhering to organization standards.

Key Responsibilities:

  • Assess veterans' needs and identify barriers in various areas.
  • Coordinate care with VA personnel and community partners.
  • Facilitate the client intake process.
  • Collaborate with clients to develop Individual Action Plans.
  • Maintain confidential client files.

Qualifications

To be successful in this role, you will require a Bachelor's Degree, preferably in Social Work, Psychology, Counseling, or a related social service field. Two years of experience in social services or a related field is also preferred. Experience facilitating support groups is a bonus.

Salary Estimate: Based on industry standards and market rates, the estimated salary for this position is $ $60,000 per year, depending on location and experience.
