About the Role: We are seeking an experienced Registered Nurse (RN) to join our team as a Palliative Care Nurse - End of Life Specialist. This is a rewarding opportunity to make a difference in the lives of patients and families facing terminal illness and bereavement.

Job Description: The successful candidate will be responsible for providing coordination of care, ensuring continuous assessment of each patient's and family's needs, and implementation of the interdisciplinary plan of care. They will also supervise nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses, and Volunteers per the plan of care.

Responsibilities:

  • Complete initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement
  • Provide palliation of end-of-life symptoms
  • Identify and utilize resources, referrals such as PT, OT, Home Health, Dietary, Pharmacy, Supplies, Home Medical Equipment, and other visits/consults
  • Document at the time of the event and at the location of care unless otherwise directed
  • Provide education and training to patients, families, caregivers, and referral sources in the care of the patient, including pain management
  • Participate in patient care meetings as assigned
  • Participate in weekly team meetings
  • Provide effective communication/shift reports through written, verbal, reporting, and body language
  • Participate in bereavement support

Requirements:

  • CA RN License
  • Open to new grad but have BSN or LVN for over a year
  • Have to be RN for over a year to not be BSN hospice

Salary: $120,000 - $150,000 per annum, depending on experience.
