Job Description:
We are seeking a skilled and compassionate LPN/LVN to provide high-quality patient care in a fast-paced sub-acute environment. The ideal candidate will have at least 1 year of experience working in a similar setting and be able to work independently with minimal supervision.
Key Responsibilities:
- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
- Current licensure as a Licensed Practical/Vocational Nurse (LPN/LVN) in the state where the facility is located.
