Job Title: Licensed Practical Nurse (LPN)

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Altru Health. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The LPN will be responsible for performing and documenting assessments, ensuring health history and admission processes are completed, and providing patient care and education. The ideal candidate will have excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient care.

Key Responsibilities:
  • Assist registered nurses and other licensed practitioners in identifying patient needs and priorities of care.
  • Contribute to the evaluation of the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions.
  • Modify the plan of care in collaboration with licensed practitioners.
  • Perform other duties as assigned to meet the needs of the department/organization.
  • Function as a member of the multidisciplinary team, contributing to the implementation of an integrated patient-centered care plan.
  • Assign or delegate the responsibility for performance of nursing interventions to other persons for which that person has the necessary skills and competence to accomplish safely.
  • Identify the early warning signs of a change in a patient's condition and report appropriately.
  • Implement nursing interventions and prescribed medical regimens in a timely and safe manner and document nursing care provided accurately and timely.
  • Collaborate and communicate relevant and timely patient information with patients/families and other multidisciplinary team members to ensure quality and continuity of care.
  • Report the patient's self-report/symptoms of pain promptly according to standard operating procedure.
  • Participate in the health education approved by a licensed practitioner.
  • Assist in the discharge planning, appropriate education and counseling based on identified needs to optimize patient/family understanding.
Requirements:
  • Licensed Practical Nurse | Minnesota Board of Nursing | Prior to Start Date | HR Primary Sources
  • Basic Life Support (BLS) | American Heart Association (AHA) | Within 90 days of Start Date | Learning Management System
Working Conditions:

This position requires occasional sitting, frequent standing, walking, stooping, bending, reaching, and handling/finger/feel. The ideal candidate will be able to lift, carry, push, and pull medium-weight objects. The work environment is primarily indoor with occasional exposure to extreme temperatures.

This is an infrequent driver position.
