- Develop and implement individualized plans of care based on patient needs, working closely with physicians and other healthcare professionals.
- Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
Requirements:
- Current RN licensure in the state of MA
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
