We are seeking a highly skilled and experienced Per Diem Hospitalist to join our team at Yale New Haven Health. As a Per Diem Hospitalist, you will be responsible for providing high-quality patient care, leading medical teams, and contributing to the development of our hospital's medical programs.
Key Responsibilities- Provide Direct Patient Care
- Admit adult patients to the hospital for private primary care physicians, from the outpatient center, and when there is no local primary care physician.
- Conduct comprehensive examinations and evaluations, and make treatment decisions.
- Communicate regularly and effectively with primary care physicians who care for the patients.
- Provide general internal medicine consultation to non-internal medicine physicians regarding patients seen in the Emergency Department and/or for pre-operative assessment, risk stratification, and post-operative co-management.
- Provide primary and consultative in-patient medical care on patients admitted to Medicine, Surgery, Obstetrics, Gynecology, and Psychiatry with appropriate sub-specialty consultation.
- Take call as necessary to provide 24/7 coverage.
- Coordinate care of patients through appropriate collaboration with other physicians, nursing staff, case managers, and ancillary service providers.
- Teach and Role Model
- Provide leadership in the Department of Medicine and the Internal Medicine Residency Program as an expert in in-patient medical practice.
- Directly supervise and teach medical residents and students who participate in the care of patients on team care.
- Role model excellence in service to patients and primary care physicians and non-internal medicine specialists and medical sub-specialists.
- Provide leadership to the Medical Staff in the use and in-service education of Physician Computerized Order Entry and the Health Information System.
- Participate and lead in formal medical education teaching venues such as teaching rounds, morning report, and conferences.
- Provide in-service education on hospital-based best practice to nursing staff.
- Assist the Residency Program Director in revising the curriculum for the residency in in-patient medical practice and consultative medicine.
- Complete mandatory hospital/unit-based in-services.
- Help identify goals for achievement consistent with the Department and Hospitals long and short-range goals.
- Actively participate in own performance appraisal.
- Demonstrate the ability to accept constructive criticism by showing improvement within established time frames.
- Evaluate Outcomes of In-patient Medical Care
- Identify problem-prone clinical practices and initiate solutions to maximize desired outcomes.
- Work collaboratively with the Hospitalist Director and Case Management and Quality Management staff in Process Improvement studies in in-patient medical practice and documenting outcomes.
- Evaluate outcomes and identify improvements in care.
- Perform and coordinate clinical documentation audits to assure compliance with best practice standards and appropriateness of medical care.
- Enhance academic and clinical productivity through publication and presentation of quality-focused PI projects locally and nationally.
- Coordinate such studies with the Internal Medicine Residency and other providers in the Yale New Haven Health System.
- Demonstrate Leadership Skills
- Work collaboratively with the Hospitalist Director to foster collegial relationships with primary care providers and enhance the Hospitalist program physician referral base.
- Work collaboratively with the Hospitalist Director on new in-patient programs and services for the Hospital.
- Work collaboratively with the Nursing staff to insure that a safe, efficient, and patient-friendly environment is maintained.
- Utilize effective time management skills. Organize day efficiently and meet organizational deadlines.
- Participate on Hospital committees and CQI teams.
- Meet volume goals set by the Hospital.
- Maintain cutting-edge knowledge and skills in hospital-based medical practice through regular attendance at CME meetings, participation in national hospital medicine and internal medicine professional organizations.
- Ensure Patient Satisfaction
- Assure that all patients are treated with respect and that needs are met promptly.
- Show consideration for patients' family and visitors by acknowledging them promptly and speaking in a way that shows concern.
- Remain calm in stressful situations, promoting a sense of confidence to patients and family members.
- Submit charges on a daily basis for services rendered.
- Attend to medical records (both electronic and paper) on a weekly basis.
