Job Responsibilities:
As a Patient Care Coordinator, you will be directly responsible for managing a designated group of patients.
Your duties will include conducting thorough assessments as defined by regulatory standards.
Develop and implement necessary adjustments to personalized care plans
Deliver the highest standard of care tailored to each patient's condition and needs.
It is essential to maintain timely and accurate documentation of care and supervisory visits in accordance with regulatory guidelines and agency policies.
Additional Duties:
Utilize community resources as necessary and maintain effective communication with the entire healthcare team.
Conduct comprehensive assessments during each patient visit.Ensure precise and current patient clinical records; accurate tracking of supply usage; and detailed daily records of patient visits, including time and mileage.
Communication:
Coordinate and communicate patient care with patients, caregivers, and all members of the healthcare team across various settings: inpatient, outpatient, medical clinics, home care, and hospice.
Education:
Engage with the Education / Performance Improvement Regional Manager in the creation and execution of educational materials and programs for the healthcare team, patients, families, and the community.
Assist in educating clinical staff through case discussions, joint visits, and structured educational initiatives.
Provide patient and caregiver education on disease management and skilled care in the home environment.
Job Knowledge:
A graduate of an accredited nursing program is required; a Bachelor's degree in Nursing is preferred.
Must hold a current license from the Mississippi Board of Nursing.
A minimum of two (2) years of nursing experience is required.
Current Basic Life Support (BLS) certification is mandatory.
Proficiency in computer skills and system connectivity is preferred.
Must demonstrate knowledge of growth and development principles across the lifespan and possess the skills to provide care to neonatal, pediatric, adolescent, adult, and geriatric patients.
