Aveanna is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN PRN Weekends. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.
Responsibilities:
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Provide skilled nursing care, including wound care, infusions, catheter care, and PICC line dressing changes.
- Educate patients and their families on disease management, medication adherence, and self-care techniques.
- Coordinate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress, including medical interventions, measurable goals, and outcomes.
Requirements:
- Active RN license in the state of application.
- Valid CPR and driver's license.
- Medicare Skilled Nursing experience preferred.
- Basic understanding of OASIS.
- 1-year RN experience in a healthcare setting.
