LPN Job Description

LakeHouse Senior Living is a leading provider of senior care services, operating 36 communities throughout the Midwest. We are committed to delivering exceptional care and services to our residents, and we are seeking a skilled LPN to join our team.

Job Summary

The LPN will provide nursing care to our residents, including comprehensive health and psychosocial evaluations, monitoring of their condition, health promotion, and prevention coordination of services. The ideal candidate will have a strong background in geriatric care and be able to work effectively in a fast-paced environment.

Responsibilities
  • Provide nursing care to residents, including administering medications, wound care, and vital sign monitoring
  • Conduct comprehensive health and psychosocial evaluations to identify resident needs and develop individualized care plans
  • Monitor resident conditions and report any changes or concerns to the Director of Nursing
  • Coordinate health promotion and prevention services, including education and support for residents and their families
  • Collaborate with other healthcare professionals to ensure seamless care delivery
Qualifications
  • Two years of LPN experience required
  • Two years of experience in assisted living or nursing home settings
  • Licensed Practical Nurse license in good standing
  • Ability to ensure community follows all federal, state, and local laws and regulations
  • Strong problem-solving and decision-making skills
  • Effective organization, time management, and communication skills
