About the Role:
As a Pregnancy Support Specialist at Great Lakes Bay, you will play a vital role in providing exceptional care to pregnant patients and their families. This is a unique opportunity to make a meaningful impact in the lives of those we serve.

In this position, you will work as part of a multidisciplinary care team to assess individual strengths and needs, utilizing knowledge and skills associated with cultural competency to achieve trust and build supportive relationships with patients. You will also be responsible for assisting with initial client/patient intake and assessment, identifying healthcare and social service needs, and participating in care team meetings to discuss patient care and exchange relevant information.

Key Responsibilities:
  • Provide guidance and support for patients needing assistance in the health plan application process
  • Communicate updated information on current outcomes or achievement related to outlined care plan goals or other agreed-upon objectives for healthy outcomes
  • Assist patients in building their individual capacity to address health and social issues
  • Support the patient's needs through education coordination along with the care team to meet the patient's medical needs

Requirements:
- High School graduate or equivalent
- Graduate of MDHHS-approved doula training program or must complete within 3 months of hire
- Current MDHHS doula certification (if already completed a training program prior to hire) specializing in birth, postpartum, and/or bereavement doula services
