Job Title: Case Manager Clinician

We are seeking a highly skilled and compassionate Case Manager Clinician to join our team at Hope Network Behavioral Health. As a Case Manager Clinician, you will play a vital role in providing comprehensive psychosocial assessments, individual and group therapy, and case management services to our clients.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments to identify client needs and develop individualized treatment plans.
  • Provide individual and group therapy sessions to clients, focusing on cognitive-behavioral techniques, coping skills, and stress management.
  • Coordinate and ensure client involvement in the Person-Centered Planning Process (PCP).
  • Meet with clients face-to-face as deemed medically necessary to assess their needs and provide support.
  • Develop and implement individual plans of service for each client, including coordination of care and communication with internal and external stakeholders.
Requirements:
  • Master's degree in Social Work, Psychology, or Counseling required.
  • Must possess or be in the process of obtaining one of the following licenses: Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW), Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), or Temporary Limited Licensed Psychologist (TLLP), Limited License Counselor (LLC) or Licensed Professional Counselor (LPC).
