Key Responsibilities- Provide geriatric house calls to older adults in their homes, delivering proactive and evidence-based therapy to improve their functional abilities and quality of life.
- Develop and implement individualized treatment plans to address the unique needs and goals of each patient.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Conduct thorough assessments and evaluations to identify areas of strength and weakness, and develop targeted interventions to address any deficits.
- Provide education and support to patients and their families on disease management, self-care, and wellness strategies.
- Valid Physical Therapy license in the state(s) of practice, or eligibility to apply.
- Degree from an accredited physical therapy program.
- Basic computer literacy skills.
- Current CPR certification.
- Ability to wear PPE, including masks, face shields, goggles, gloves, and gowns.
- Integrity, compassion, and enthusiasm.
