Essential Functions:
- Contribute to teamwork by consistently offering assistance and responding positively to requests.
- Provide guidance to the interprofessional healthcare team and apply feedback to enhance patient outcomes.
- Utilize therapeutic communication techniques, including active listening and teaching, to build relationships with patients and families.
- Collaborate with team members by effectively communicating and reporting relevant patient care information.
- Educate nursing and support staff, orient new team members, and assist in the training of students and clinical fellows.
Service Responsibilities:
Conduct thorough evaluations of patients across various diagnoses to identify their functional levels and determine appropriate treatment plans. Develop and implement individualized physical therapy treatment plans for complex patient populations while practicing in alignment with cultural diversity and inclusion principles.
Patient Advocacy: Act as an advocate for patients and families to address clinical and ethical concerns, utilizing available resources.
Quality and Safety Functions:
Regularly assess patients, monitor progress towards treatment goals, and modify plans as necessary. Identify opportunities for improvement and contribute to meeting departmental and hospital targets for quality and safety.
Documentation: Maintain accurate and timely documentation of patient education and procedures performed, ensuring compliance with established standards.
Professional Development: Engage in self-development activities and participate in departmental projects and community service opportunities.
