Physical Therapist Assistant

We are seeking a dedicated and compassionate Physical Therapist Assistant to join our team of skilled home health care providers.
Job SummaryUnder the direction of a Physical Therapist, the Physical Therapist Assistant provides client services that have been delegated by the Physical Therapist.
Responsibilities- Participates in the preparation of clinical and daily progress notes, all notes are reviewed by the Physical Therapist and submitted according to company policy.
- Performs routine strengthening and therapeutic exercises and assists the patient with the use of self-help devices.
- Instructs the client and family and assists in teaching exercise programs to the patient and family.
- Identifies possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and reports same to the Physical Therapist.
- Observes, and records activities and findings in the clinical record and reports to the Physical Therapist and the physician any adverse reaction to treatment.
- Consults with Physical Therapist regarding the outcome of home visits and reports physical findings.
- Instructs other health team personnel including, as appropriate, Home Health Aides, as well as family members with respect to certain phases of physical therapy, so that they may work with the client.
- Writes daily progress notes on patients, and submits according to company policy, and prepares periodic summary reports as indicated.
- Participates with staff, clients, and physicians in discharge activities and assists the Physical Therapist in completing the Physical Therapy Discharge Summary.
- Confirms, at least weekly, the scheduling of visits with the Supervisor/Director of Nursing in order to coordinate Agency visits.
- Assumes other duties deemed appropriate and necessary by the Physical Therapist and as designated by Registered Nurse Case Management and Nursing Supervisor.
Graduate of an accredited Physical Therapy Assistant program or equivalent education.
Minimum of one year's experience in home healthcare preferred.
Electronic medical record experience is preferred but not essential.
Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
