Join Our Team as a Primary Care Provider for Assisted Living Communities
CentraCare is seeking a dedicated and compassionate Primary Care Provider to join our team in providing whole-person care to seniors in assisted living communities throughout our service area. As a Primary Care Provider, you will be responsible for providing on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities.
Key Responsibilities:
- Provide on-site primary care to patients within Assisted Living and Memory Care communities
- Develop and implement care plans appropriate to patients and their desires and geriatric best practices
- Collaborate with community partners within home care, hospice, and other resources as appropriate
- Support from scheduling and coordination assistants and clinical support from LPNs and RNs
- Access to diagnostics such as labs and x-rays
Requirements:
- Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
- Board Certified/Board Prepared through AANP, ANCC, or NCCPA
- Licensed or ability to obtain licensure through the MN Board of Nursing
- Ability to obtain and/or maintain DEA certification for the State of Minnesota
- Proven ability to communicate effectively with strong verbal skills
- Excellent interpersonal and customer service skills
