Key Responsibilities:- Provide high-quality, patient-centered care to individuals in their homes and communities.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Provide education and support to patients and their families to promote healthy behaviors and self-care.
- Current State of Maine RN licensure.
- Associate Degree in Nursing Field.
- Experience in home health and hospice care is a plus.
