About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a key member of our care team, you will work with patients in their homes to evaluate, direct, and provide speech/language pathology services.

Responsibilities
  • Evaluate patients' speech and language abilities and develop personalized treatment plans
  • Provide one-on-one therapy sessions to patients in their homes
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and communicate with healthcare teams
  • Participate in care coordination activities and discharge planning
Requirements
  • Master's degree in Speech Language Pathology or related field
  • Current certification in Speech-Language Pathology or Audiology
  • Minimum of six months experience as a speech therapist or speech-language pathologist
  • Home Health experience preferred
  • Current CPR certification
  • Good organizational and communication skills
  • Valid driver's license and reliable transportation
