We are a mobile primary care, palliative care, and hospice service provider serving patients in various regions. Our clinicians provide in-home care for patients with chronic conditions and limited mobility. We offer a range of programs, including remote patient monitoring, behavioral health management, and chronic care management, to ensure our patients receive high-quality care from a trusted team.

Job Summary

We are seeking an experienced PRN Hospice Provider to care for homebound patients in the Manasquan, NJ region. The ideal candidate will have strong clinical skills, a positive demeanor, and the ability to work independently. Responsibilities include examining patients, assessing their health, and developing treatment plans.

Key Responsibilities

  • Provide high-quality care to primary and palliative patients in the Manasquan, NJ area
  • Assess patient health through physical examinations and medical histories
  • Effectively communicate with office staff and clinical operations
  • Develop treatment plans and provide patient-centered care
  • Attend medical staff meetings and order diagnostic testing as necessary
  • Prescribe medications and interventions as needed

Requirements

  • Board Certified with a valid NP certification in New Jersey
  • Strong clinical skills and ability to diagnose and treat patients
  • Positive demeanor and strong people skills
  • Ability to work independently and problem-solve
  • 1-2 years of Hospice care experience with geriatric patients (Hospice experience preferred)
