Job Summary:
We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and developing personalized care plans to improve their overall well-being.
Responsibilities:
- Conduct thorough health assessments, including medical history, diagnosis, and treatment, to identify potential health risks and develop personalized care plans.
- Collaborate with healthcare teams, including physicians, case managers, and social workers, to ensure seamless care coordination.
- Provide education and support to patients and their families to promote healthy behaviors and improve health outcomes.
- Work in a variety of settings, including patients' homes, skilled facilities, and telehealth environments.
Requirements:
- Master's degree in Nursing or related field.
- Current RN and NP licensure in the state of practice, with prescription authority.
- Board certification in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with electronic health records.
- Excellent communication and interpersonal skills.
