Job SummaryWe are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
Responsibilities- Conduct comprehensive health assessments in the home setting
- Develop and implement individualized care plans to address patient needs
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy behaviors and disease management
- Document all patient interactions and care provided in accordance with Matrix policies and procedures
- Masters Degree in Nursing required
- Current RN and NP licensure in state of practice
- Board certified by AANP, ANCC, or AACCN in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
