We are seeking a highly skilled and compassionate Healthcare Navigator to join our team at Proactive MD. As a key member of our clinical operations, you will play a crucial role in facilitating continuity in applicant and employee care, ensuring seamless transitions between client occupational health services and community provider networks.

About the Role

This exciting opportunity involves coordinating care services within provider networks, imaging centers, labs, and community resources to deliver exceptional patient satisfaction and outcomes. You will work closely with our Clinical Operations Manager, Client Relations team, Network Providers, and Client HR Team to develop and implement effective case management plans that meet the unique needs of each client and their employees.

Responsibilities

Your key responsibilities will include:

  • Coordinating candidate and employee care services to ensure seamless transitions and high-quality outcomes;
  • Ensuring medical records are provided to Proactive MD following visits and sending them to employers as required;
  • Collaborating with the Clinical Operations Manager and Client representative to develop employer Occupational Health case management plans;
  • Demonstrating exemplary customer service in coordination and communication surrounding Health Center visits;
  • Celebrating cultural diversity and providing patient-centered care to individuals from diverse backgrounds;

Requirements

To succeed in this challenging yet rewarding role, you will need:

">
  • A minimum of 2 years of experience working in an Occupational healthcare setting or similar role;
  • Knowledge and understanding of OSHA medical surveillance requirements and process;
  • Current BLS certification through American Heart Association valid for at least 90 days after start date;
  • Strong communication and interpersonal skills, with the ability to build and maintain relationships with people from diverse backgrounds;
  • Ability to prioritize tasks and manage multiple projects simultaneously;

Salary and Benefits

We offer a competitive salary range of $55,000 - $65,000 per year, depending on experience, along with a comprehensive benefits package that includes health insurance, retirement savings, and paid time off.
