Job Summary:

The Phoenix Center for Rehabilitation and Nursing is seeking a part-time Occupational Therapist to join our team. As an Occupational Therapist, you will be responsible for providing diagnostic and therapeutic services to evaluate, treat, document, and/or facilitate care for residents with impaired activities of daily living due to disease process. You will participate in interdisciplinary team meetings as required and assist the Director of Nursing Service to develop ADL decline prevention policies and procedures.

Key Responsibilities:

Evaluate and treat residents with impaired activities of daily living
Develop and implement individualized treatment plans
Document resident progress and outcomes
Participate in interdisciplinary team meetings
Assist in developing ADL decline prevention policies and procedures

Requirements:

Master's degree in Occupational Therapy
Current Occupational Therapist license
At least two years of related experience
Excellent verbal and written communication and organizational skills
Ability to effectively communicate with residents, their families, and other healthcare professionals
