Key Responsibilities- Assessment and Intervention: Conduct thorough assessments of psychosocial factors, mental and emotional health, and stability to develop effective treatment plans.
- Case Management: Provide direct case work services, including counseling and therapeutic interventions at the individual and group level.
- Communication and Collaboration: Communicate goal progression and plan of care to patients, staff, and physicians, while working collaboratively with clinical/medical staff, office manager, and community resources.
- Documentation and Budgeting: Document all patient care activities in a timely manner and manage patient care fund budget, expenditures, and record keeping.
- Education: Master's Degree in Social Work or Social Work Education.
- Licenses and Certifications: LCSW (Licensed Clinical Social Worker) or LMSW (Licensed Medical Social Worker) in the State based on facility location.
- Work Experience: Minimum of one year experience in a healthcare setting.
- Knowledge, Skills, and Abilities: Must be knowledgeable of basic medical terminology, major medical diagnoses, and principles of growth and development over the life span.
