Job Summary

We are seeking a skilled Registered Nurse to join our Case Management team at Ascension Health. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare settings.

Responsibilities
  • Develop and implement comprehensive care plans that address the unique needs of each patient.
  • Collaborate with healthcare providers to ensure timely and effective care delivery.
  • Monitor patient progress and make adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making.
Requirements
  • Current RN licensure in the state of Oklahoma.
  • 3+ years of experience in a healthcare setting, preferably in case management or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
