Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse. As a key member of our care team, you will be responsible for providing high-quality patient care, coordinating care plans, and collaborating with healthcare professionals to ensure the best possible outcomes for our patients.

Responsibilities
  • Conduct skilled nursing visits and coordinate patient care plans
  • Serve as the primary point of contact for all disciplines involved in patient care
  • Oversee the frequency of visits for each patient episode
  • Consult with physicians and office staff to ensure accurate and timely patient care
  • Collect and document accurate OASIS data, including medication profiles and lab values
  • Attend regular case conferences and case manager meetings to ensure seamless care coordination
Requirements
  • Current licensure as a Registered Nurse (RN) in the state of employment
  • A minimum of one year of clinical experience preferred
  • Demonstrated knowledge and skill in current nursing practice
  • Valid state driver's license and automobile liability insurance
