Responsibilities:
Assess patients' needs and develop individualized care plans in collaboration with the patient care team and the patient/family.
Provide patient care that reflects a respect for patients' rights, dignity, values, culture preferences, and expressed needs, and an attention to providing comfort, relieving pain, and alleviating anxiety.
Conduct utilization management assessments to facilitate appropriate use of healthcare resources and coordination of care.
Collaborate with patient/family, physician, hospital staff, and community agencies to develop discharge plans that prepare patients for continued care needs.
Implement care in accordance with the plan, or clinical protocol, utilizing care team members in the care delivery process.
Identify variances from clinical protocols, resolve or refer issues as appropriate, and identify opportunities for the improvement of care.
Respond appropriately to changes in workloads, patient census, and/or staffing levels.
Assess patient/family needs for information and education across the continuum, plan and implement patient teaching using a variety of techniques and methods, and evaluate effectiveness of educational interventions.
Administer ordered medications and treatments as ordered by physicians.
Monitor patients for side effects of treatments, coordinate with physician, and implement supportive care measures.
Perform phlebotomy and laboratory on-site testing, record test results, and communicate test results to the physician.
Responsible for patient medical record documentation in accordance with policies and procedures.
Triage patients, follow established protocols, and assure appropriate advice is given to patients and families.
Participate in staff meetings, education in-service programs, and complete required continuous training and education.
Implement quality control programs by reviewing and logging all quality controls and acting appropriately when results read out of control limits.
Requirements:
Graduate of an accredited school of nursing or equivalent.
Current licensure to practice nursing in the State of Michigan.
Current Basic Life Support (BLS) certification.
Oncology Certified Nurse (OCN), preferred.
Three years of RN experience with two years of experience in oncology.
