Job Title: Nurse Practitioner - PRN/Part Time

About the Role:

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner, you will be responsible for conducting comprehensive health assessments in the comfort of patients' homes, providing high-quality care and improving health outcomes.

Key Responsibilities:

  • Conduct thorough health assessments, including medical history, medication adherence, and social environment
  • Develop and implement individualized care plans to address patients' needs
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families

Requirements:

  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • 1+ year experience as a Nurse Practitioner
  • Excellent communication and interpersonal skills
