Home Health Physical Therapy Assistant

LHC Group is seeking a skilled and compassionate Home Health Physical Therapy Assistant to join our team in Augusta, Georgia.
About the Role:As a Home Health Physical Therapy Assistant, you will play a vital role in providing high-quality care to patients in their homes. Your primary responsibility will be to assist the supervising Physical Therapist in developing and implementing treatment plans that promote patient independence and well-being.
Key Responsibilities:- Assist the supervising Physical Therapist in conducting patient assessments and developing treatment plans.
- Provide exercise instruction and patient education to promote patient independence and well-being.
- Observe and record patient responses to treatment and report any changes in patient condition to the supervising Physical Therapist and other healthcare team members.
- Coordinate care with other healthcare team members to ensure seamless patient care.
- Instruct patients, family members, caregivers, and other healthcare team members on therapy techniques and patient care.
- Current licensure, registration, or certification to assist in the practice of physical therapy in the state of practice.
- Ability to work independently and as part of a team.
