We are seeking a highly skilled RN Care Manager to join our team at Intermountain Healthcare. As a key member of our healthcare team, you will work collaboratively with physicians and other healthcare professionals to provide exceptional patient care and improve health outcomes.
Key Responsibilities- Conduct comprehensive assessments of patients' health needs, including health status, behaviors, level of function, psychosocial situation, and available support systems.
- Develop and implement individualized care plans in collaboration with the primary care physician and patient care team.
- Provide health education and coaching to patients and families to empower them to manage their health care needs.
- Identify patients at risk for proactive intervention and develop strategies to prevent hospitalizations and emergency department visits.
- Collaborate with payers and outside agencies to promote patient-centered delivery concepts and improve health outcomes.
- Participate in committees and activities related to the development of Medical Home, Accountable Care Organization (ACO), and other insurance carriers.
- Associates Degree in Nursing (ADN/ASN)
- Current RN license in state of practice
- Minimum of two (2) years of clinical experience in an outpatient setting or a minimum of two (2) years of experience coaching individuals with complex and/or chronic conditions
- Bachelors Degree (BS/BA/BSN)
- Prior experience in care coordination in a community setting or primary care practice
- Program development, operations management, and/or supervisory experience
This position requires the ability to:
- See and read information, labels, assess patient needs, operate monitors, identify equipment and supplies
- Frequent interactions with patient care providers, patients, and visitors that require verbal communication, hearing, and understanding spoken information, alarms, needs, and issues quickly and accurately
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy
- Lift and utilize full range of movement to transfer patients
- Walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment
