Supplemental Health Care is seeking a skilled Physical Therapist to join our team in College Park, Georgia.

About the Job

We are looking for a dedicated Physical Therapist to work with our home health care provider in College Park, Georgia. As a Physical Therapist with Supplemental Health Care, you will have the opportunity to work with a variety of patients and provide high-quality care in a dynamic and supportive environment.

Job Responsibilities
  • Provide physical therapy services to patients in their homes
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and maintain accurate records
Requirements
  • Graduate of an accredited Physical Therapy program
  • Licensed as a Physical Therapist in Georgia
  • CPR certification or ability to obtain
  • Previous experience with computer charting
  • 6 months of previous home health care experience
  • Valid U.S. driver's license and reliable transportation
