Job DescriptionThis role offers an excellent opportunity for a registered nurse to grow their skills and experience while providing high-quality care to patients in their homes.
Duties and Responsibilities- Develop and implement individualized care plans for patients based on their unique needs and goals.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Evaluate patient outcomes and adjust care plans as needed to ensure optimal results.
- Document all interactions and interventions accurately and thoroughly.
- Meet regulatory requirements and maintain compliance with agency policies and procedures.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
$60,000 - $80,000 per year, depending on experience.
