Nurse Care Specialist

We are seeking a skilled and compassionate Registered Nurse to join our team at Geisinger Home Health. As a Full-Time RN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver comprehensive care.
Key Responsibilities:
- Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop individualized plans of care.
- Provide and direct provisions of nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
- Coordinate total plans of care with other healthcare professionals involved in patient care, ensuring continuity and effective communication.
- Maintain accurate, complete, and timely documentation in collaboration with physician orders.
Requirements:
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
