Nurse Care Manager

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Manager. In this role, you will be responsible for providing high-quality healthcare services to individuals with intellectual and developmental disabilities.
Key Responsibilities- Conduct comprehensive assessments to identify healthcare needs and develop individualized care plans.
- Collaborate with physicians and external medical centers to ensure accurate and timely communication of medical information.
- Provide technical assistance and medical oversight to staff to ensure effective implementation of healthcare services.
- Participate in the development of individual service plans and provide input on healthcare services.
- Monitor and ensure that physicians' orders are documented and carried out as prescribed.
- Travel between multiple locations to provide support and be accessible to staff.

- Graduate of an accredited RN program.
- One year of experience working with the population we serve.
- Current state RN License.
- Current driver's license, car registration, and auto insurance.
- Current CPR/First Aid certification.
- Excellent assessment skills with the ability to exercise critical thinking in the clinical environment.
- Effective communication skills, with the ability to communicate clearly with team members, medical professionals, and individuals and their families.
