About the Job

We are seeking a compassionate and skilled Home Health Aide to join our team. As a Home Health Aide, you will provide patients with in-home personal care and designated health-related services to maintain their physical and emotional well-being.

Responsibilities
  • Follow current written aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs) such as mobility transfer, walking, grooming, bathing, dressing or undressing, eating, or toileting.
  • Perform incidental household services essential to the patient's healthcare at home that are necessary to prevent or postpone institutionalization.
  • Complete a clinical note for each visit within required timeframes, which must be incorporated into the patient's record.
  • Provide care according to the aide plan of care/assignment sheet and in accordance with State Practice Act or regulations.
Qualifications
  • Successful completion of a competency evaluation.
  • Current CPR certification required.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
