Occupational Therapist Position in Sub-Acute/Skilled Nursing Facility

We are seeking a skilled Occupational Therapist to join our team in a sub-acute/skilled nursing facility setting. As an Occupational Therapist, you will provide comprehensive and compassionate therapy services to our residents.
Key Responsibilities:- Provide individualized therapy services to residents with a focus on improving functional abilities and promoting independence.
- Collaborate with other rehab therapists and nursing staff to develop and implement effective treatment plans.
- Conduct thorough assessments and evaluations to identify resident needs and goals.
- Develop and implement individualized therapy plans to meet resident needs and goals.
- Document resident progress and maintain accurate records.
- Current Occupational Therapist (OT/OTR) license or eligibility to practice in the state of employment.
- Strong computer skills with Microsoft Outlook, Word, Excel, and experience with electronic documentation.
- Excellent communication and interpersonal skills.
- Ability to work well in a team environment and provide compassionate care to residents.
