Key Responsibilities:

  • Deliver clinical services, including gathering patient information, leading therapeutic activities, contributing to treatment plans, and ensuring thorough documentation of all services rendered.
Manage and coordinate physician appointments and laboratory tests within the designated program or facility.

Essential Duties:

  • Assist the treatment team by overseeing patients in the care environment, actively participating in the execution of the multidisciplinary treatment strategy, recognizing significant changes in patients' health status, and communicating these observations to relevant team members.
  • Uphold customer service and performance enhancement objectives of the department; collaborate with colleagues across various departments to improve patient care and service delivery.
  • Organize physician schedules in accordance with established protocols for routine visits, accommodating changes due to patient needs, and collaborating with program leadership and clinicians for patient triage. Document and bill for each physician visit.
  • Collect laboratory specimens as directed by the physician.
  • Gather health information from patients as required.
  • Facilitate individual and group educational and activity sessions.
  • Record each treatment session in compliance with documentation standards.
  • Engage in treatment rounds, working alongside the treatment team to coordinate care through the exchange of clinical insights.
  • Maintain professional and respectful interactions with patients, families, and staff in all daily responsibilities.
  • Participate in departmental performance improvement initiatives and objectives.
  • Adhere to confidentiality standards.
  • Possess the necessary knowledge, skills, and attitudes to address patients' physical, psychological, and social needs across various developmental stages and patient demographics served within the department.
  • Contribute to unit compliance with established standards.
  • Undertake additional duties and projects as assigned.

Qualifications:

  • Licensed Practical Nurse (Required)
  • Active Practical Nurse License in the state of New Jersey

Educational Requirements:

  • High School Diploma or GED (Required)
  • Completion of a nursing program as an LPN. Previous experience with the program's patient demographic is preferred.
