Home Health Occupational Therapist for Premier Excellence

FeldCare Connects, an innovative referral network, seeks a dedicated Occupational Therapist to deliver exceptional care to adults of all ages. This position is ideal for an Independent Contractor serving La Mesa and surrounding areas.
The Occupational Therapist in Home Health will conduct thorough assessments and evaluations to determine patient care needs, including functionality status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment, the Occupational Therapist develops a personalized treatment plan, implements interventions to enhance patient wellbeing, and evaluates progress.
Key Responsibilities:
- Provide services defined by state laws governing Occupational Therapy, in accordance with the plan of care, and in coordination with the healthcare team.
- Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
- Observe, record, and report patient responses to treatment and changes in condition.
- Instruct patients, families, caregivers, and the healthcare team in areas of therapy where they can assist the patient.
Qualifications:
- Ability and enthusiasm in working with diverse age groups.
- Occupational Therapist license and registration by the state.
- Completion of an accredited Occupational Therapist program.
- Bilingualism is highly valued.
