Nurse Navigator

Mass General Brigham is seeking a skilled Registered Nurse (RN) to fill the role of Home Hospital Navigator in our Emergency Department. As a key member of our team, you will be responsible for identifying eligible patients for transfer to our Home Hospital program and ensuring a seamless transition of care.
Responsibilities:- Collaborate with the Home Hospital team to identify patients who meet eligibility criteria for transfer to our Home Hospital program.
- Conduct clinical and social eligibility and safety screening to ensure patients are suitable for transfer.
- Communicate with the Emergency Department care team, patients, and families to facilitate transfers and ensure a smooth transition of care.
- Work closely with the Home Hospital Admitting Provider and Case Manager to ensure timely and efficient transfers.
- Utilize analytical and critical thinking skills to make informed decisions and judgments that align with organizational objectives.
- Current RN licensure in the state of Massachusetts.
- Minimum 3 years of experience in emergency department, home hospital, or inpatient settings.
- Varied experience preferred.
