We are seeking an experienced Advanced Practice Provider to join our team at Summit Health, Inc. in Dallas, Texas. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our physicians and staff to deliver exceptional patient outcomes.

Responsibilities
  • Provide comprehensive and coordinated care to adult patients, including health history and physical assessments, diagnosis, and treatment planning.
  • Work autonomously and make clinical decisions without physician supervision.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with our multidisciplinary team to ensure seamless care transitions and optimal patient outcomes.
  • Stay up-to-date with the latest clinical guidelines and best practices in geriatric medicine.
Requirements
  • Graduate of an accredited advanced practice program.
  • Current, unrestricted license to practice as an Advanced Practice Provider in the state of Texas.
  • Active DEA registration.
  • Medicare/Medicaid enrollment.
  • Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification.
  • Strong communication and interpersonal skills.
  • Experience with Electronic Medical Record (EMR) systems, preferably Athena.
