Responsibilities- Provide comprehensive and coordinated care to adult patients, including health history and physical assessments, diagnosis, and treatment planning.
- Work autonomously and make clinical decisions without physician supervision.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with our multidisciplinary team to ensure seamless care transitions and optimal patient outcomes.
- Stay up-to-date with the latest clinical guidelines and best practices in geriatric medicine.
- Graduate of an accredited advanced practice program.
- Current, unrestricted license to practice as an Advanced Practice Provider in the state of Texas.
- Active DEA registration.
- Medicare/Medicaid enrollment.
- Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification.
- Strong communication and interpersonal skills.
- Experience with Electronic Medical Record (EMR) systems, preferably Athena.
