About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager. In this role, you will provide direct care to patients and coordinate with the interdisciplinary team to ensure a seamless care experience.

Key Responsibilities:

  • Conduct comprehensive assessments of patients and families to identify their unique needs
  • Develop and implement individualized care plans to meet the medical and emotional needs of patients and families
  • Administer medication and treatment as prescribed by the physician
  • Provide education and support to patients and families to promote self-care and independence
  • Collaborate with the interdisciplinary team to ensure continuity of care
  • Ensure compliance with all state and federal regulations and guidelines

Requirements:

  • Graduate of an accredited nursing school with current licensure to practice in the state of operation
  • 1 year of nursing experience required, 3+ years preferred
  • Experience in hospice or a similar setting preferred
  • Knowledge of the hospice philosophy of care
  • Commitment to clinical and documentation excellence
