Job Summary

We're seeking a highly motivated and experienced Case Manager I to join our team. In this role, you'll work closely with patients, families, and healthcare professionals to provide comprehensive care and support. Your expertise will help patients achieve their goals and improve their overall well-being.

Key Responsibilities
  • Develop and implement individualized treatment plans in collaboration with patients, families, and healthcare professionals.
  • Provide education and support to patients and their families on mental health topics, including DBT, CBT, and REBT.
  • Coordinate care with interdisciplinary teams, including LCSWs, psychiatrists, and other healthcare professionals.
  • Monitor patient progress, identify areas for improvement, and adjust treatment plans as needed.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Manage crisis situations and provide support to patients and their families during times of need.
Requirements
  • Master's degree in a non-clinical field (e.g., social work, counseling, or psychology).
  • Experience working in mental health settings, preferably in a case management or clinical coordination role.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Knowledge of psychiatric diagnosis, terminology, and medical record charting basics.
  • Ability to work in a fast-paced environment and manage multiple priorities simultaneously.
  • Valid license in good standing (e.g., LCSW, LMFT, or LPCC).
