As a Nurse Practitioner, you will have the opportunity to make a positive impact on the lives of elderly patients and contribute to a supportive care team.

• Provide primary and preventative care to long-term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Develop and implement advanced care plans and chronic care management strategies.
• Prescribe medications, including controlled substances, in accordance with CO Law.
• Order, review, and follow up on treatments and diagnostic testing.
• Communicate regularly with nursing home staff to discuss changes in condition and care plans.
• Collaborate with primary care physicians to ensure comprehensive patient care.

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family, or geriatric specialties.
• Experience working in long-term care settings.
• Ability to work with electronic medical records.
• Ability to manage patients with advanced illnesses and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
