Estimated Salary: $60,000 - $80,000 per year

As a key member of our rehabilitation team, you will work closely with patients to develop personalized care plans tailored to their unique needs. Your expertise will play a crucial role in helping patients regain independence and achieve optimal health outcomes.

 

Key Responsibilities:

  • Conduct thorough assessments of patients' medical conditions and functional abilities
  • Develop and implement comprehensive rehabilitation plans to address patients' physical, emotional, and cognitive needs
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
  • Maintain accurate and timely documentation of patient progress and treatment plans

 

Requirements and Qualifications:

To succeed in this role, you must possess:

  • A degree in a relevant field, such as occupational therapy, physical therapy, or speech-language pathology
  • Current licensure and certification in your state and profession
  • Proven experience in rehabilitation settings, preferably with geriatric or adult populations
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient groups and healthcare professionals
