Psychosocial Specialist for Dialysis Clinic Patients

This position provides critical psychosocial services to patients undergoing dialysis treatment at our clinic. As a Psychosocial Specialist, you will collaborate closely with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for our patients.
Responsibilities- Assess patients' psychosocial status, strengths, and areas of need that may affect their rehabilitation and treatment outcomes.
- Collaborate with patients and healthcare teams to identify effective interventions using established social work theory and methods, patient education programs, and quality of life measurement instruments.
- Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
- Assess patients' knowledge of kidney disease and identify barriers to adherence with treatment.
- Offer supportive counseling services to patients within the scope of your clinical training and state license.
- Provide information and assist the team and patient with referrals to community resources to facilitate optimal treatment outcomes.
- Maintain current knowledge of local vocational/educational rehabilitation programs and assist patients with referral and access.
- Assess patient awareness of advance directives and facilitate discussion of advance directive wishes, if necessary.
- Report on quality indicators related to adherence and service recovery.
- Work with patients, families, and healthcare teams to provide education tailored to the patient's learning style and needs.
- Facilitate transplant referral processes and collaborate with interdisciplinary teams on transplant waitlist management.
- Provide ongoing education to patients/families regarding psychosocial issues related to end-stage renal disease and support services.
- A Master's degree in Social Work is required, along with a state-required license.
- This role requires 0-2 years of experience in a related field.
- Fresenius Medical Care North America offers a competitive salary range of $71,000 - $105,000 annually, depending on qualifications, location, and experience.
