Responsibilities:- Provide professional nursing care to residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician.
- Conduct resident assessments, administer medications and treatments as ordered, and provide technical support to healthcare professionals as needed.
- Document resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary.
- Counsel residents, family members, and/or resident advocates on prevention and treatment plans, under the direction of a registered nurse, advanced practice provider, or physician.
- Develop and implement self-care programs for residents, utilizing knowledge of resident education principles.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Associate degree in practical nursing is preferred.
- Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum or have had a minimum of 400 hours of experience as LPN at another facility.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
