We are seeking a skilled Registered Nurse to join our team at Avera@Home Sioux Falls. As a Nurse Care Coordinator, you will play a vital role in delivering high-quality patient care in our patient-focused settings.
Key Responsibilities- Provide nursing care, care coordination, lab draws, nebulizer treatments, and other necessary cares for patients during their acute stay.
- Collaborate with multidisciplinary teams to ensure comprehensive care and excellent patient outcomes.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Graduate from a CCNE or NLN accredited nursing program is strongly preferred.
- Bachelor's degree in nursing preferred.
- Minimum 2 years related experience and/or training; or equivalent combination of education and experience preferred.
- License as a Registered Nurse (RN) in South Dakota and/or meeting the requirements of Multi-state licensure is required.
- Competitive hourly rate of $44.25 per hour.
- Varied shifts, including every third weekend and occasional call.
