Job Summary

As a Licensed Practical Nurse on our dialysis team, you will play a vital role in coordinating and implementing patient care plans. Your expertise will ensure that our patients receive the highest level of care and attention.

Key Responsibilities
  • Conduct thorough pre, intra, and post-dialysis assessments to identify patient needs and develop effective care plans.
  • Administer medications and provide documentation of administration and results.
  • Prepare and monitor hemodialyzers and hemodialysis machines, performing quality control checks as required.
  • Obtain laboratory specimens and perform in-house laboratory procedures in accordance with facility policies and procedures.
  • Review lab reports, identify problems, and report to Charge Nurse and Registered Nurse as needed.
  • Complete monthly nursing assessments and assist in reviewing and co-signing assessments performed by non-licensed personnel.
  • Assist in patient review of long-term care plans and document patient education as necessary.
  • Review and document patient consent forms and facility policies and procedures upon admission and annually.
  • Prepare and perform transfusion patient monitoring and quality control checks for emergency carts and kits.
  • Participate in administrative and staff programs, including staff meetings, company in-services, and CQI meetings.
  • Transcribe and implement physician orders.
  • Support and promote positive attitudes and loyalty to management.
  • Maintain a clean and orderly work area and respond to emergency situations.
  • Assist in teaching and training new staff members as directed by Charge Nurse, Registered Nurse, or Nurse Manager.
Requirements
  • Current licensure as a Licensed Practical Nurse in the state where the site is located or licensure in a compact state.
  • CPR certification.
  • Completion of an accredited Licensed Practical Nurse program.
  • At least 2 years of clinical and hemodialysis experience.
  • Demonstrated record of safety and commitment to excellence.
  • Outstanding communication and interpersonal skills.
  • Ability to read and interpret documents, including safety rules, operating instructions, and procedure manuals.
  • Computer skills, including data entry, navigation, and basic troubleshooting.
