Job Title: RN Home Health Nurse

The Home Health Registered Nurse is a skilled professional responsible for providing and documenting individualized patient care in accordance with physician-ordered plans. This role involves delivering high-quality care in the patient's home or residential setting, with occasional transfers to other locations for appointments and activities.

Key Responsibilities:
  • Develop and revise patient care plans in collaboration with patients, families, physicians, and care teams.
  • Complete and submit OASIS assessments, reassessments, and other necessary documentation within Agency-defined timeframes.
  • Process orders, notify physicians of patient needs, and complete certification/recertification orders and discharge summaries.
  • Provide a respectful and confidential environment for patients, their families, and property.
  • Initiate preventive and rehabilitative strategies, and provide specialized nursing services as needed.
  • Consult with and educate patients, families, and team members on disease processes, self-care techniques, and preventive strategies.
  • Supervise LPNs to ensure patient health, safety, and compliance with care plans.
  • Direct, coordinate, evaluate, and supervise patient care services provided by the Home Health Agency.
  • Interact with patients, physicians, referral sources, and others in a collaborative and customer-focused manner.
  • Support Care Management philosophy, including collaborative planning and case management to meet individual health and service needs.
  • Maintain accurate and complete clinical records for compliance with state and federal legislation and seamless service delivery.
  • Stay current with available URMHC and community-based services.
  • Ensure problem resolution at the point of contact and authorization/release of information forms are signed by patients or responsible parties.
  • Attend patient care conferences for assigned patients.
Requirements:
  • Certified Home Health Care Experience Required
  • EPIC or Netsmart Required
